How to Make Your Selections

To add a product, check the box under Your Order. The total price will be calculated at the bottom of the form. Please add local sales tax. An $8.00 shipping & handling fee will be added to physical orders.

How to fill out our form using Google Sheets

If you’d like edit online using Google Sheets, make a copy you can edit online in Google Docs, go to File menu in Google Sheets > Make a Copy. See image for location—the menu is in the web browser window and not the Windows or Mac File menu.
